The following are the basic requirements for motorcar claims:

    For Own-Damaged Claim
  1. Policy must be fully paid.
  2. Copy of the insurance policy.
  3. Police report or notarized affidavit.
  4. Repair Estimate.
  5. LTO Official Receipt and Certificate of Registration.
  6. Valid driver’s license with Official Receipt.
  7. Clear pictures of damaged part of the vehicle (include plate no.)
    For Third Party Property Damaged
  1. Same requirements as above.
  2. Certificate of No Claim from the Insurance Company of Third Party (CTPL / Comprehensive)
    For Third Party Bodily Injury
  1. Police report.
  2. Hospital’s Statement of Account with charge slips.
  3. Original official receipts for medical, hospital or doctor’s bills.
  4. Physician’s prescription and Drugstore Invoices of medicines or supplies.

    For Carnapped/Stolen Vehicles
  1. Policy must be fully paid.
  2. Copy of the insurance policy.
  3. Police report, Complaint Sheet, Alarm Sheet.
  4. LTO Official Receipt and Certificate of Registration.
  5. Valid Driver’s license and Official Receipt.
